Scientists Say Creatine May Help Fight Depression
A recent systematic review has explored creatine’s possible role in alleviating depression symptoms, but the findings are inconsistent. While some trials indicate benefits, others show no significant impact, suggesting further research is necessary.
Evaluating Creatine’s Role in Depression
Creatine, primarily recognized as a supplement for enhancing muscle performance, is now being examined for its potential mental health benefits. Researchers, led by Bassam Jeryous Fares from the University of Ottawa, conducted a systematic review assessing whether creatine could alleviate depression by bolstering the brain’s energy supply. The review synthesized results from five randomized controlled trials, encompassing 238 individuals across multiple countries.
Study Design and Sample Characteristics
The included studies varied in design, focusing on different populations and methodologies. Overall, 126 participants received creatine, while 112 were given a placebo. Most participants were women, with two studies comprising only female subjects. Among these trials, four concentrated on individuals with major depressive disorder, while one involved participants with bipolar disorder experiencing a depressive episode.
Inconsistent Findings Across Trials
The review yielded mixed outcomes regarding the efficacy of creatine in treating depression. In two of the studies involving women with major depressive disorder, participants taking five grams of creatine daily, alongside standard antidepressant treatment or cognitive behavioral therapy, exhibited significant reductions in depressive symptoms compared to those receiving placebo. One trial reported a large effect size, reflected by a Cohen’s d of 1.13.
Conversely, three other trials did not identify meaningful advantages of creatine over placebo. One study indicated that neither five nor ten grams of creatine daily improved outcomes for individuals whose depression was resistant to medication. Another trial noted no benefits in adolescent girls, and a third, which included participants with bipolar disorder, also failed to demonstrate improvements.
Safety and Future Research Directions
While creatine appears generally safe, with mild gastrointestinal issues noted as adverse events, the review highlights potential safety concerns. Two participants with bipolar disorder developed hypomania following treatment with creatine. The authors stress that the existing body of evidence does not yet support the routine use of creatine for depression treatment, as most studies had limitations, such as small sample sizes and varying quality.
The researchers advocate for larger and more rigorous studies extending beyond the eight-week duration observed in current trials. Additionally, they suggest exploring the combination of creatine with exercise and varying doses to determine potential benefits more effectively. Notably, animal studies indicate that responses to creatine may differ by sex, which could explain the more favorable results observed in female participants in human studies.
